Benefits for seniors in Oklahoma
In short: In Oklahoma, Medicaid is called SoonerCare. The Oklahoma Health Care Authority runs it, and Oklahoma Human Services (OKDHS) decides who qualifies if you are 65 or over, blind or disabled. The help many seniors miss is the Medicare Savings Programs, which can pay your Medicare Part B premium and sometimes much more. For free, unbiased Medicare help, call the Oklahoma Insurance Department's Medicare Assistance Program at 800-763-2828.
Medicaid for people 65 and over
SoonerCare is Oklahoma's Medicaid program. If you are 65 or over, blind or disabled, you apply through OKDHS.
- Income: Countable income must be under $1,350 a month for one person, or $1,824 for a couple. This is 100% of the federal poverty level, the same limit used for QMB below.
- Savings and other assets: $2,000 for one person, or $3,000 for a couple.
- What it covers: Doctor and hospital care, prescriptions and other medical services. If you also have Medicare, SoonerCare can help with costs Medicare leaves behind.
Long-term care. For nursing home care or the ADvantage home care program, the income limit is higher: $2,982 a month for one person. The asset limit stays the same. A DHS nurse checks whether you need this level of care. In a nursing home you keep $75 a month for personal needs, and you may have to pay part of your income toward your care. If your spouse still lives at home, they can keep between $32,532 and $162,660 in assets, and may keep income up to $4,067 a month. Your home equity cannot be over $752,000.
Estate recovery. If you got long-term care at 55 or older, the state may seek repayment from your estate after you die, but not while a spouse is alive or while a child age 20 or younger, or a disabled child living in the home, survives. A hardship waiver is possible.
Help paying Medicare costs
OKDHS runs the standard Medicare Savings Programs (MSPs). You must have Medicare Part A.
- QMB (Qualified Medicare Beneficiary): Pays your Medicare premiums, deductibles and copays. Income up to $1,350 a month for one person, or $1,824 for a couple.
- SLMB (Specified Low-Income Medicare Beneficiary): Pays your Part B premium. Income up to $1,616 a month for one person, or $2,184 for a couple.
- QI-1 (Qualifying Individual): Pays your Part B premium. Income up to $1,816 a month for one person, or $2,455 for a couple.
Asset test: Yes. Assets must be under $9,950 for one person, or $14,910 for a couple.
Oklahoma uses the federal limits. Some income is not counted, so apply even if you think you are a little over.
If you get any MSP, you also get Extra Help with drug costs automatically.
Prescription drug help
The main help with drug costs in Oklahoma is the federal Extra Help program. It lowers your Part D premium and copays. You can get it if your income is below 150% of the federal poverty level, about $23,940 a year for one person in 2026.
OKDHS has local Medicare help coordinators (called MIPPA coordinators) who can help you apply for Extra Help and the MSPs. Ask the Senior Info-Line at 1-800-211-2116 for the one near you.
Extra state payment with SSI
Yes. Oklahoma pays a State Supplemental Payment (SSP) to people who get SSI, or would qualify, because of age or disability. OKDHS runs and pays it, not Social Security, so it comes separately from your SSI check.
- How much: Up to $40 a month for one person living in the community. The exact amount is the state standard of $1,034 minus your countable income.
- Total with SSI: The federal SSI payment is up to $994 a month.
- Assets: The same limits as SSI, $2,000 for one person.
- Apply for SSI first: If you are 65 or older with no income, you must apply for SSI with Social Security and be approved before OKDHS can approve the SSP.
Food help
SNAP food benefits are run by OKDHS.
- Medical costs count: If you are 60 or older or disabled, tell OKDHS about medical costs you pay yourself. Bring proof, such as 60 days of pharmacy printouts, insurance premium statements, medical bills and transportation costs. These can raise your benefit.
- Savings and home: OKDHS says bank accounts, homes and vehicles do not affect SNAP eligibility.
- Meals: Group meals at senior centers, and home-delivered meals for people 60 and over who need them. There is no income test. Contact your Area Agency on Aging.
- Senior Farmers Market Nutrition Program: OKDHS and some tribal nations (Chickasaw, Choctaw and Osage) give low-income people 60 and over coupons for fresh fruits and vegetables at farmers markets each summer. Call OKDHS at 405-522-5050 to ask about this year's season.
- Commodity Supplemental Food Program (CSFP): A monthly food box for low-income people 60 and over. It is not offered in every county. Call 405-522-5050.
- EBT card help: 1-888-328-6551.
Help staying at home
- ADvantage waiver: For people 65 and over (and some younger adults with disabilities) who need nursing home level care but want to stay at home. It covers case management, personal care, nursing, home-delivered meals, medical supplies, prescriptions, home changes, adult day health and assisted living. You must qualify for SoonerCare and meet the medical rules. Apply online through OKDHS, or call the Medicaid Services Unit at 1-800-435-4711 to apply by phone. Ask them about current wait times.
- State Plan Personal Care: Help with bathing, dressing, meals, laundry, shopping and light housework for SoonerCare members who need it. Same phone number.
- PACE: For people 55 and over who need nursing home level care and live in a PACE area. Oklahoma has PACE in Tulsa, Oklahoma City, Shawnee and Tahlequah. Ask the Oklahoma Health Care Authority at 405-522-7044.
- Area Agencies on Aging: Eleven agencies offer free help for people 60 and over, such as chores, homemaker help, transportation, legal help and home repair. Call 1-800-211-2116.
Property tax and housing help
- Property valuation limitation (for 65 and over): This can hold down increases in the taxable value of your home. The head of household must be 65 or older on January 1. The combined income of everyone in the home must be under a limit set each year for your county. For tax year 2026 the county limits run from $60,600 to $99,900. Return Form 994 to your county assessor by March 15, or within 30 days after you get a notice that your value went up, whichever is later.
- Homestead exemption: If you own and live in your home on January 1, the first $1,000 of its assessed value is not taxed. File Form 921 with your county assessor by March 15 (or within 30 days of a notice that your value went up). You only file once, unless you move or your ownership changes.
- Additional homestead exemption: Another $1,000 of assessed value is exempt if your gross household income was $30,000 or less last year. Use Form 994, the same form as the valuation limitation, by the same deadline.
- Property tax refund (Form 538-H): If you are 65 or older (or totally disabled), you are the head of household, and your total household income was $12,000 or less, you may get back part of the property tax on your home, up to $200. File with the Oklahoma Tax Commission by June 30 for last year's taxes. Attach a copy of your tax bill.
- 100% disabled veterans: The full value of the home is exempt for Oklahoma veterans rated 100% disabled from service, and for a surviving spouse who has not remarried. File Form 998 with your county assessor.
Ask your county assessor's office which of these you can get.
Help with energy bills
OKDHS runs the Low Income Home Energy Assistance Program (LIHEAP). It has three parts, each open for a short time while money lasts:
- Winter heating, crisis help and summer cooling: In 2026, sign-up opened in early January, mid-April and mid-July.
- Life-threatening crisis help: Open all year if someone in the home needs life-saving medical equipment, or during extreme heat or cold. Call 405-522-5050 and choose the energy assistance option.
Income: For the summer 2026 sign-up, monthly gross household income had to be $1,696 or less for one person, or $2,292 for two. You must be responsible for the energy bill. Apply online at okdhslive.org when sign-up is open. Have your latest utility bill, photo ID, Social Security number and proof of income ready. Tribal members can also apply through their tribal nation.
